Teletask, Inc: Text Message Verification Requirements
In order to use Teletask for sending text messages, we have
to submit the phone number for validation. This applies for both toll free and
local numbers. The validation process was created by the phone providers to be
in compliance with the FCC
regulations
regarding sending messages to participants. Participants are required to opt in
to receiving text messages. They can opt in either through a physical form that
is signed, are verbally asked, or through texting in a keyword.
Local or Toll Free Texting Phone Number
LOA form – if using your own number. Otherwise this can be skipped.
EIN/FID number (typically found on your W-2 or tax exemption form):
- Legal Name (associated with tax number, also in W-2):
- Address (associated with tax number, also in W-2):
- Entity Type (government, non-profit, or corporation):
- Opt-in Process (Form, verbal, or keyword, see below):
- Form if applicable or list of keywords:
- Screenshot of where opt-in answers are recorded:
- Link to your site's Privacy Policy where the Messaging Privacy Policy and Messaging Terms & Conditions can be found (see below):

Opt-in Options
The form must include the following text as a minimum along with a check box for opting in to messaging:
I want to receive text messages from [Organization Name] regarding appointment reminders, benefits, educational materials, surveys or to be able to communicate back and forth directly with staff members. I understand that message and data rates may apply and message frequency may vary. I can text HELP for help or text STOP or contact a staff member to unsubscribe at any time. Your phone number will not be shared with third parties for marketing or promotional purposes. The privacy policy can be found on at: [website link].
B. Verbal Script:
The script must include the following as a minimum:
Participants are verbally asked "Do you want to receive text messages from [Organization Name] regarding appointment reminders, benefits, educational materials, surveys or to be able to communicate back and forth directly with staff members?"
Their answer is recorded [link to image where the yes/no is indicated].
If yes, participants are told "Please note that message and data rates may apply and message frequency may vary. You can text HELP for help or text STOP or contact a staff member to unsubscribe at any time. Your phone number will not be shared with third parties for marketing or promotional purposes. Our privacy policy can be found on our website: [website link]
C. Keyword:
The keyword response text must include the following as a minimum:
Initial message will state: Thank you for opting in to receive recurring messages from [Organization Name]. Msg frequency varies. Msg & data rates may apply. Reply HELP for help. Reply STOP to cancel. Your phone number will not be shared with third parties for marketing or promotional purposes. [Link to Privacy Policy site]
Website Privacy Policy and Messaging Terms & Conditions
Regardless of the opt-in option, it is required that your website contain at minimum the following entries:
Messaging Privacy Policy
We collect phone numbers from users. Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. If you wish to be removed from receiving future communications, you can opt out by texting STOP.
Messaging Terms & Conditions
You agree to receive informational messages (appointment reminders, educational materials, surveys, two-way conversations, etc.) from [Organization Name]. Message frequency varies. Message and data rates may apply. For help, text HELP or call us at [phone number]. You can opt out at any time by replying STOP.
