How to: Create a Web Survey Report

The Web Survey report type allows you to view the results of your Web Survey. You can run the report on a specific survey or a specific time frame.
Note:   Web Surveys must be enabled on an account prior to use. Please reach out to Teletask staff to enable this feature.

Create a Web Survey Report

    1. Log-in to: https://hms.teletask.com/, your email address is your username.

    2. Click on Reports   in the top menu bar.

    3. Select Web Survey  from Reports section.

    4. Choose which Web Survey that you want to run the report for.

    5. Optional: Limit the report results by participants that belong to particular Clinics or choose All clinics to include all participants.



    6. Enter your Starting and Ending Date Range for this report.



    7. If the Survey is being tracked by contact info, decide if you would like to view the Responses by Contact or Responses by Question.

      Note: If the survey is anonymous, this option will not be available and all results will display Responses by Questions.


      1. For Responses by Contact, decide what columns you want to include in the report by selecting Change Report Columns.



    8. Press the Generate Report button  to view the results.

      Note: To see more questions, scroll to the right or download the results into a .csv file.

    9. Optional: Click the response Link to view any image files that are attached.
    Once the results are generated, you can export them as a .csv Excel file  or choose to print them .
