Note: Web Surveys do not offer the ability to use a clients preferred language. A separate survey can be created in another language and then send to clients who use that preferred language.
Note: Web Surveys must be enabled on an account prior to use. Please reach out to Teletask staff to enable this feature.
- Log-in to: https://hms.teletask.com/, your email address is your username.
- Click on Messages
in the top menu bar.
- Press Create Message
button in the top right-hand corner.
- Click the Select button for Add a Web Survey.
- Decide if you want to Keep responses anonymous. or if you want to Track responses by contact info.
- Enter a Name for the new survey. This name is for your reference and the name of the message that will appear under the Messages section.
- Enter a Title for the survey. The Title can include additional details regarding the survey such as specifying the purpose of the survey or details about filling it out.
- Choose Add Question
to start entering questions for the survey.
- Type your question in the provided box.
Remember: include what ratings mean such as a 1 is the best answer and 5 is the worst if applicable.- Select the Question Type you will be using for this specific message.
- Click the Add Response Option
to add answer options for the question.

This message will look like this to the client. Choosing the +Add another Item will add a new text box to enter additional information.- OPTIONAL: Check the Required box to validate that that question must be completed before the survey can be submitted.
This will add a red star to a message denoting that it is required.- Once you have finished adding your questions and answers options, select Save
at the top of the screen to keep the changes.
A dialog box will appear with your Survey URL. ClickClose.Note: This Survey URL is for anonymous tracking only.
Note: Once a Web Survey is created, it will appear in the Web Surveys section of Messages.

Open or create the message that you are going to include the Web Survey in.Note: Surveys should be added to Text or Email messages only.- Under the Contents section of the message, select Start or Edit under Text or Email.
- Type the message that you want to include with the survey. Leave your mouse curser at the spot where you want the Web Survey link to appear.
- Use the Add Web Survey drop-down, on the right-hand side, to select the survey you want to send. This will create a URL for the web survey. If the survey is set to track responses, it will create a dynamic URL with a customized ending when sent. If the survey itself is already set to anonymous, it will not track the results.
- The Merge Tag for the survey will appear in the content of the message.

Email Message Example:- Finish making the rest of your changes to the message and send it to your desired participants.
- When the participant clicks on the link, the survey questions will display.
- When you select the choose file option, you can either take a photo or add an existing photo.
- After they click submit they will receive this message.
